Closing the Gap Riverland is a team of Nurses and Aboriginal Outreach Workers people who work with Aboriginal clients to coordinate access to adequate, preventative and comprehensive primary health care services.
Closing the Gap Riverland has a focus on prevention, including the promotion of healthy lifestyles for all ages and the early detection and management of chronic disease.
Through our integrated programs, we assist Aboriginal people to identify their health needs, develop a care plan and coordinate their access to services.
All Closing the Gap Riverland services are free or low cost, anyone who identifies as Aboriginal or Torres Strait Islander can become a client. Clients can self-refer by calling RDGP on 08 8582 3823, or by dropping into the office at 3, Riverview Drive Berri. Health professionals can refer clients by completing a Referral Form and faxing it to RDGP on 08 8582 5050.
